1. Limited to quantities of $50 gas cards on hand based on a first come first served basis.
2. Past referrals prior to 1/1/2007 are not eligible for the $50 gas card.
3. Only 1 gas card will be given for each new vehicle that contracts with the SECAT program. There is no limit on the number of vehicle referrals.
4. Fill out additional copies of Page 1 of the referral form if more than 3 companies are being referred to the SECAT program.
5. Dealers and SMAQMD employees are not eligible for the $50 gas card.
6. Existing or past participants of the SECAT program with funded projects do NOT qualify as referrals.
7. All information provided in this SECAT Referral Form will be used by SMAQMD to evaluate the eligibility of the referrer to receive a $50 gas card for each vehicle he/she referred to the SECAT program. SMAQMD staff reserves the right to request additional information and can void the referral form if such requested information is not provided. The SMAQMD will at its sole discretion determine if the referrer meets the criteria for receiving a $50 gas card. Invalid referral forms will be discarded once the referrer has been notified.
